Position Summary:  The Licensed Practical Nurse provides comprehensive health care services to designated clients, working collaboratively with contracted and referring physicians and other physical health providers. 

Position Qualifications:

  • A LPN diploma and PA licensure along with two years related experience.
  • Valid PA Driver’s license and access to a reliable.
  • Ability to be flexible and can multi-task functions.
  • Able to handle confidential information according to HIPAA guidelines.
  • Strong computer skills and experience with electronic health records for documentation purposes.
  • Support the organization’s mission along with sensitivity of cultural and workplace harmony.

Essential Duties and Responsibilities: 

  • Implement and maintain Familylinks’ philosophy, principles and procedures in all facets of performance.
  • Develops, coordinates, administers, and implements health care services in compliance with agency policies and procedures and all external requirements and best practice standards.
  • Assists in maintaining a safe and secure environment for staff and patients.
  • Participates in care of patients as an active part of the treatment team and works under the supervision of the physician/CRNP.
  • Assists in completing prior authorizations for medications to be approved and covered by the patient’s insurance.
  • Under the supervision of the physician/CRNP, communicates with clients (parents/guardians of clients who are minors) to discuss treatment, side effects, laboratory results, medication refills, or other clinical concerns.
  • Under the supervision/direction of the physician/CRNP administers medications as needed.
  • Upon receiving direct verbal orders or written orders from the physician/CRNP, assists in transferring orders to the pharmacy.
  • Evaluates vital signs, height, weight and waist circumference as part of a patient’s evaluation and ongoing treatment planning.
  • Maintains patient confidentiality.
  • Under the supervision of the physician/CRNP, assists in coordinating care with other behavioral health or medical providers.
  • Provides input in the assessment, evaluation, and development of behavioral or physical health services and policies.
  • Maintains accurate records and documentation of patient treatments, discussions and laboratory results according to standard medical recordkeeping.
  • Maintains all agency equipment and supplies as necessary.
  • Assists in the coordination of and participates in staffing as required.
  • Attends supervision, trainings and meetings as directed.

Working Conditions:   The LPN will be working in community-based facilities.   The work will include individuals who have complex needs, including but not limited to: mental health; substance use; communicable diseases; verbal and physical aggression.

Physical Requirements:  Active:   Lifting up to 50 lbs. maximum and occasionally carrying such articles as files, ledgers and small office tools.  Walking and standing along with the physical demands of working are required to perform the duties of the position.
