Resident Assessment Coordinator (RN)

Summary:Provides support, education and training to assigned facilities on the resident assessment instrument (RAI) process in accordance with all laws, regulations and standards.

Coordinates clinical operations in assigned facilities to ensure compliance with Medicare guidelines and managed care case management policyQualifications:Education:Must have nursing degree (or diploma) from accredited college
– RN necessaryLicenses/Certification: Nursing License for the state(s) within the RegionMDS 3.0 certificationExperience:Three years of nursing experience in a long term care environment preferredMust have 1+ year working with RAI process and PPSEssential Functions:Able to read, write, speak and understand the English languageAble to move intermittently throughout the work dayPractice regular and predictable attendanceUnderstand and follow company policiesReview MDS Detail and RUGS monthly or more frequently as needed for assigned facilitiesProvides support to assigned facilities in training of MDS staffProvides support to assigned facilities to prepare clinical team on MDS processProvides support to assigned facilities for MDS issues and concerns during survey processProvides support to assigned facilities in Identifying Medicare entitlement and eligibility for prospective residentsProvides support and training to assigned facilities on MDS softwarePerforms other duties as assignedKnowledge/Skills/Abilities:Excellent verbal and written communication skillsProficient in Word, Excel and emailHave good computer software skillsMust be willing and able to travel when neededAbility to provide education to professional and non-professional staffKnowledgeable of nursing practices and procedures as well as laws, regulations and guidelines governing nursing functions in long-term careAbility to plan, organize, develop, implement and interpret programs, goals, objectives, policies and procedures of clinical servicesAbility to work cooperatively as a member of a teamAbility to communicate effectively with residents and their family members, and at all levels of the organizationKnowledge of Medicare regulationsAbility to maintain confidentiality of all information
